Whose format this is
This is Maharashtra's format, and only Maharashtra's. The School Education and Sports Department revised the school leaving certificate and the General Register together, in a Government Resolution dated 19 September 2016. Before that, the leaving certificate's format sat in rule 17 and Appendix 4 of the Secondary School Code, and the General Register's in Appendix 18.
The GR gives its reasons plainly. A leaving certificate is an important document in a student's life, needed to join another school, to get a caste certificate or a passport, and for much else. Certificates differed from school to school, which confused students and parents, and the format had to change so that certificates could be issued through the Saral system.
It applies from the 2016-17 year to schools of every medium and every management, and it tells heads of school to print both the register and the certificate in the new format.
The fields, in English
The GR's Marathi heading is शाळा सोडल्याचे प्रमाणपत्र. These are its fields, in order. The template keeps the body fields in the GR's order and fits the header details around the letterhead, so the certificate prints on one A4 page at 12pt.

The GR prints the form only in Marathi. If your education office expects the Marathi labels, type them into the Word file in place of the English ones; the fields and their order stay the same.
How it differs from a CBSE TC
Both certificates do the same job, but they ask for different things. CBSE's is the Annexure-I transfer certificate in its Examination Bye-Laws.

If you're a CBSE school in Maharashtra, CBSE's bye-laws expect a TC in its Annexure-I format before a student is entered in a CBSE school's admission register. The 2016 GR is addressed to schools of every medium and management, and a copy went to the chairpersons of CBSE and ICSE. We didn't find anything that settles which certificate a CBSE school in Maharashtra should issue, so check with your regional office before you change what you use.
Other states, APAAR and PEN
We looked for prescribed leaving certificate formats from other states, including Gujarat, West Bengal and Karnataka, and didn't find one we could verify. If your state or board prescribes a format, use that one.
We also didn't find a later Maharashtra GR that adds the APAAR ID or the UDISE+ PEN to the leaving certificate. The template sticks to the 2016 fields.
How to fill in the leaving certificate
- Start from the General Register. Every entry comes from the student's line in General Register No. 1. Write its number at the top, next to the certificate's serial number.
- Number every certificate. Give each certificate the next serial number, and keep an office copy in the same order.
- Write the date of birth twice. First in figures by the Christian calendar, then in words.
- Fill in the leaving details. The date of leaving, the class the student was studying in and since when, in words and figures, and the reason for leaving.
- Sign and seal. The class teacher, the clerk and the headmaster or principal sign, and the school seal goes on.
